►GUIDELINES◄
• Teams can be made of up to 5 bowlers. The registration fee is $50 per team ($10 per person). A Team Captain must register the team online. Each team member is encouraged to raise at least $50 each in sponsorships as well. Ask your friends, family members, and co-workers!
• Use our sheet to sign up your sponsors who are contributing by cash or check. Credit card sponsorships can be made online at www.bowlathon.net. Once you’ve registered to participate, your sponsors can make secure online donations in your name that will be credited to your team.
• Bring your completed Sponsor Sheet on the day you bowl and turn it in with your collected cash and check donations.
• Collect your donations at the time they are made. Checks should be made payable to “HealthLink”.


All proceeds from BOWL-A-THON benefit HealthLink Medical Center.
